Hybrid Strength & Performance Waiver, Terms and Conditions, Rules and Standards
Thank you for choosing Hybrid to help you achieve your goals! 
During your exercise program, every effort will be made to assure your safety. However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. 
Hybrid Strength & Performance Terms and Conditions​
  • Training sessions that are not rescheduled or canceled at least 24 hours in advance will result in forfeiture of the session and loss of the financial investment for that session.
  • A $15 late fee will be applied seven (7) days after the invoice due date. If the invoice remains unpaid, an additional $15 will be charged each week until it is paid in full.
  • Clients arriving late will receive only the remaining scheduled session time unless prior arrangements have been made with their coach.
  • If you or a member of your household damages equipment due to negligence or misuse, you will be financially responsible for repairs or replacement.
  • All training sessions must be completed within a five-week period. Any unused sessions will be forfeited after this timeframe.
  • Group training sessions are billed on a recurring 28-day (4-week) cycle. A new invoice will be sent out every 29th day.
  • No refunds will be issued unless approved by Caleb Lindsey.
Hybrid Strength & Performance Rules & Standards
  • Hybrid Strength & Performance is a high-quality performance center focused on helping each person who walks through the door reach their full potential. Please treat the facility, its coaches, and visitors with the respect they deserve.
  • While in our facility, do not enter restricted areas or use equipment that has not been designated for your use without permission.
  • Destructive, demeaning, rude, abusive, or sexually harassing language or actions will not be tolerated. Failure to comply with this standard will result in removal from the program.
    We are here to work.
  • You have invested your time and money into the betterment of your health and athletic performance. Do not waste anyone’s time by giving less than your best effort every time we meet.
Hybrid Membership Rules & Liability
  • Hybrid Strength & Performance, LLC is not responsible for any injury sustained while training in the facility, and you assume all risks associated with unsupervised exercise.
  • You agree not to share your door codes with anyone or bring guests who have not been properly vetted and approved by Hybrid Strength & Performance, LLC.
  • You will be responsible for the repair or replacement of any equipment you damage while training at Hybrid Strength & Performance.
  • You must be 18 years or older to have unsupervised access to Hybrid Strength & Performance, LLC.
  • Respect equipment and other gym members at all times. Failure to do so may result in termination of your membership.
  • Re-rack your weights and wipe down any equipment you use.
  • Return equipment to its proper location when you are finished with it.
  • If you are the last person in the facility, turn off the lights and ensure the door closes and locks behind you.
  • Hybrid Strength & Performance, LLC groups have priority access to equipment during their designated times.
  • Hybrid Strength & Performance, LLC reserves the right to black out certain hours for large groups, Saturday bootcamps, and similar events.
  • Membership dues are billed on the first day of each month and will be considered late after seven (7) days.
  • A $15 late fee will be added to your account if payment is not received within those seven (7) days.
